The following paperwork is required for all athletes for the 2024 Season. All documents must be submitted electronically. If you are unable to submit these forms electronically.

Pop Warner Little Scholars - 2024 Participant Contract and Parental Consent Form (Required to register)
Pop Warner Little Scholars 2024 Medical History Form (Required to register)
Copy of Birth certificate - Required to Register (No hospital records) (Athletes that qualify for Nationals, must have originals ready. Details to come when needed)
Final report card for 2023-2024 - must state report card, and include all four quarters, child’s name, grade and school. Math page required if applicable. Transcripts will not be accepted.
